For each community we'll customize a survey form to help us prioritize efforts, identify contributors (time, money, advice) and solicit feedback

Section 1: Demographic Context (4 Questions)

  1. Role: Parent/Teacher/Administrator/Community Member [Dropdown]

  2. School District: Free-text with autocomplete suggestions.

  3. Grade Levels Affected: Multi-select (PreK–12).

  4. Prior Engagement: Have you contacted school officials about security? (Y/N).

Section 2: Threat Perception (6 Questions)

  1. Safety Confidence: “How safe do you believe your child’s classroom is during a lockdown?”

    • Scale: Very Safe → Extremely Vulnerable.

  2. Breach Awareness: “Were you aware that most classroom doors can be breached in under 15 seconds?” (Y/N).

  3. Current Measures: Rate effectiveness of existing security features (metal detectors, lockdown drills).

Section 3: Solution Support (8 Questions)

  1. Funding Priority: “Should classroom doors be upgraded before other security investments?” (5-point scale).

  2. Donation Likelihood: “Would you contribute $50?” (Y/N/Maybe).

  3. Advocacy Channels: Opt-in to email legislators, join town halls, or share on social media.

Section 4: Open Feedback (2 Questions)

  1. Concerns: “What barriers prevent your community from adopting this solution?” (Free-text).

  2. Success Stories: “Describe a time when community action improved school safety” (Qualitative).
